Features of Revised Crash Report Form

Major features of the new form include:

A reduced number of data elements by removing the old form elements that were not used.

A graduated approach in which the same form can be used as either the "short form" (pages1 and 2) or "long form" (pages 3 or 4) investigated crashes.

Two tear off copies of operator and vehicle information for distribution to drivers.

Consistency with the new Agency of Transportation Crash Reporting System database and national data standards for crash reporting (including commercial vehicle crashes).

Improved ease and ability to record crashes involving commercial motor vehicles.

Streamlined overlay design to facilitate efficient use.

The forms come in pads of 10 with overlays stapled in place for ease of use in the field.
